Absolutely stunning and extremely practical. As a young person with multiple chronic conditions, hospital appointments become the 'norm' but can be extremely confusing. This book is going to be a lovely way of keeping track of things and is gorgeous and far from clinical.

This book's idea is to keep all your details conveniently in one place. So, for appointments or hospital visits, you have all your medical info on hand. The book also notes your nutrition, and your support needs, and even emergency contacts like next of kin. If you dial 999, paramedics can read info about you and what essential items they need to grab from your home. Plus, there's space for notes and care plans. It covers mental health too, with dedicated pages on symptoms and helpful tips for medical settings, including the importance of good sleep.
The aim is for it to be a holistic resource for anyone with health conditions to store their medical information. And hopefully to make life a bit easier.

*Please note, there's a carefully worded page on "getting nutrition" suitable for most people with eating problems. Near the back, you'll find an optional page to track weight, which is helpful for doctors prescribing certain medications. This can be torn out, but you would lose the page on adaptations for dressing, toileting, and bathing. The book doesn't mention BMI. I hope I've met as many people's needs as possible.
